Google Pixel Watch 5 Breathing Emergency Detection Uses On-Device AI to Call for Help

Google on Wednesday introduced the Pixel Watch 5 alongside the Pixel 11 smartphone family, with a new health feature called Breathing Emergency Detection, according to WIRED. The feature uses an on-device AI algorithm to detect a persistent drop in oxygen saturation and automatically call emergency services, sharing the wearer's location. It is the latest addition to Google's expanding Health Guardian suite, following the first-of-its-kind Loss of Pulse Detection feature introduced on the Pixel Watch 3.

What Is Breathing Emergency Detection?

According to WIRED, the feature can be triggered by a variety of factors, including drug toxicity, accidents, severe pneumonia, and choking. It relies on data from a combination of advanced sensors: a PPG (photoplethysmography) sensor, an accelerometer, and a barometer. The AI algorithm runs on-device and detects a persistent drop in oxygen saturation, then calls Emergency Services and shares the wearer's location.

"Using a combination of data from the advanced sensors, including a PPG, accelerometer, and barometer, we've designed an AI-based algorithm running on device to detect a persistent drop in oxygen saturation and then call Emergency Services for you and even share your location," said Francis Hoe, product manager on Google's Pixel Watch team.

Regulatory Status and Availability

WIRED reported that Google has not received Food and Drug Administration (FDA) clearance for Breathing Emergency Detection, so it will not launch in the United States at this time. Google said it is working with regulators to expand availability, and the feature will be available only in select markets in the European Union to start. The company also stated it is not intended for people with preexisting respiratory conditions and is not designed to diagnose or treat medical conditions.

Google is bringing the feature to the Pixel Watch 4 as well, so existing users do not need to upgrade to the Pixel Watch 5 to take advantage.

Health Guardian Suite Expands

Google is placing these new capabilities into an umbrella category called Health Guardian, according to WIRED. This includes existing features such as irregular rhythm notifications, high/low heart-rate notifications, Loss of Pulse Detection, and Sleep Breathing Quality. The design mirrors Google's Personal Safety app on Pixel phones, which consolidates Emergency SOS, Safety Check, and Car Crash Detection in one place.

New in the suite are blood pressure and insulin resistance trends, delivered through the Google Health app. WIRED reported these will be available for anyone with a Fitbit Air, Pixel Watch 3, Watch 4, or Watch 5 later this fall. Many Health Guardian features will deliver monthly reports based on 24/7 passive monitoring, with Google offering suggestions for action. Users can either consult the new AI-powered Health Coach or send their data to their physician.

How the AI Models Were Built

Underlying these features is years of opt-in sensor data collection. Sandeep Waraich, senior director of Wearables at Google, told WIRED: "It might have felt strange—why are we harping about heart rate accuracy so much—back then. We kept investing in that, and now it's very improved, because all these large sensor models can detect these signatures, insulin resistance, blood pressure trends—but underneath, they need a really capable raw signal."

Rishi Chandra, vice president for Google Health, explained that Google has collected opt-in PPG signals from Fitbit users for many years, giving the company an understanding of different health conditions. He said that people with elevated blood pressure have a unique signature in the data, which can be matched to an arm cuff for prediction.

Competitive Context

WIRED noted that Apple debuted hypertension notifications for select Apple Watch models last year, and Samsung offers blood pressure monitoring on its smartwatches, though it requires monthly calibration with a cuff. Google's blood pressure and insulin resistance trends are designed to help people proactively monitor these potential health issues before it's too late, as high blood pressure is the leading risk factor for cardiovascular diseases.
